02 · PLAN YOUR QUESTIONS
Prepare for a school conversation
Use the School Conversation Question Guide to prepare questions about reading data, instruction, interventions, evaluations, and follow-up communication.
03 · PUT CONCERNS IN WRITING
Prepare a written evaluation request
Use the Written Evaluation Request Template to organize your concerns and prepare a written request to discuss a comprehensive evaluation with your child’s school.
Need help choosing your next step?
The Dyslexia Pathway Navigator uses your answers to organize guidance, resources, and next steps for your family. Use it when you want more context than a single template provides.
These tools support preparation and communication. They do not diagnose dyslexia or determine a child’s eligibility for services. The Navigator’s guidance is designed for Texas families.
Using your parent resource templates
How do I download a template?
Open the English or Spanish version. Select Print or Save as PDF, then choose your printer or your device’s PDF option. You can print the blank template and complete it by hand.
When should I use the Navigator instead?
Use a template when you already know the task you want to complete. Use the Dyslexia Pathway Navigator when you want guidance on what to do next. Sign in is available for parents returning to saved pathways.
